- If you purchase a one hour consultation, she will send you medical history forms as well as a legal disclaimer form within 24 hours of your purchase. You'll send her back all documentation as soon as you can. She needs to review it prior to your consultation. After your consultation, you will receive a summary email.
- You can use your H.S.A. card if desired. Telemedicine is now being widely approved by many insurance companies. Dr. Crawford is happy to supply you with documentation to apply for insurance reimbursement in the U.S.

In late 2016 I talked with you on the phone and told you about my condition.
